The findings are chilling: a staggering 589 cases during the pandemic and 369 post-pandemic, with a mean age of around 32 years.

And this is the part most people miss: while ethanol (the type of alcohol in beverages) was consumed more frequently, methanol, a highly toxic substance often found in illicit alcohol, was the real killer. Methanol poisoning had a mortality rate of 29.4%, compared to just 8.6% for ethanol. Even more concerning, methanol victims were far more likely to suffer permanent visual impairments (8.1% vs. 1.1%).

The study identified key risk factors:

  • Methanol Consumption: The single biggest predictor of death, with a fourfold increased risk compared to ethanol.
  • Lack of Pre-Hospital Care: Patients arriving at hospitals via non-emergency transport faced higher mortality rates, highlighting the critical need for immediate medical intervention.
  • Delayed Treatment: The time between ingestion and hospital arrival played a significant role in outcomes.

Interestingly, being male was associated with a slightly lower risk of death, though the reason remains unclear.

The root cause? Misinformation. During the pandemic, false claims circulated that gargling or drinking alcohol could prevent COVID-19. This, coupled with Iran's ban on alcohol driving people to the black market, created a deadly cocktail.
